Nurse Practitioner
with Diabetes Experience
25 Miles NW of Milwaukee
The Practice:
The Nurse Practitioner of the Endocrine and Diabetes practice will
be responsible for providing patient care and education for Diabetes
and Primary Care patients in collaboration with our 75 + primary
care and specialty physicians.
Responsibilities:
• Evaluation and management of patients with diabetes, following
the National Standard for Diabetes Self-Management Education (DSME)
as defined by the American Diabetes Association (ADA).
• Ordering, interpreting and responding to changes in patient
diagnostic testing.
• Teaching and counseling patients and families in the areas
of preventive care, self-management, injection training, insulin
pump management, and plan of care based on diagnosis as defined
by the American Diabetes Association (ADA).
• Participating in organization sponsored community support
groups and educational offerings.
• Developing and evaluating patient educational materials
and care management programs.
• Acting as an organizational resource to inpatient or outpatient
staff, physicians and leadership on insulin pump management.
Qualifications:
• APRN WI License
• DEA License or able to option license
• CDE preferred but not required
• Must have 3-5 years experience in diabetes management
