	Nurse Practitioner with Diabetes Experience 25 Miles NW of Milwaukee The Practice: The Nurse Practitioner of the Endocrine and Diabetes practice will be responsible for providing patient care and education for Diabetes and Primary Care patients in collaboration with our 75 + primary care and specialty physicians. Responsibilities: • Evaluation and management of patients with diabetes, following the National Standard for Diabetes Self-Management Education (DSME) as defined by the American Diabetes Association (ADA). • Ordering, interpreting and responding to changes in patient diagnostic testing. • Teaching and counseling patients and families in the areas of preventive care, self-management, injection training, insulin pump management, and plan of care based on diagnosis as defined by the American Diabetes Association (ADA). • Participating in organization sponsored community support groups and educational offerings. • Developing and evaluating patient educational materials and care management programs. • Acting as an organizational resource to inpatient or outpatient staff, physicians and leadership on insulin pump management. Qualifications: • APRN WI License • DEA License or able to option license • CDE preferred but not required • Must have 3-5 years experience in diabetes management The Community: • Outstanding community with a rolling landscape and remarkable atmosphere for family living, business, and industry. 	Unique CDE opportunity Practice Therapeutics is currently partnering with Novo Nordisk to utilize Certified Diabetes Educators to deliver a diabetes management program. The focus of this program is to assist health care practitioners to improve their management of diabetes. This is a full time field based position. The Educator will conduct education regarding diabetes standards of care to primary care office staff and physicians to assist in better management of their diabetes patients. CDE position available in the Green Bay and surrounding areas. Ideal candidate will possess: •RN or RD with four year degree •Current certification as a Certified Diabetes Educator required •Valid driver’s license in good standing in the state in which the Clinical Educator will be located Essential Responsibilities and Competencies: •Conduct educational programs in primary care offices •Coordinate program implementation with office staff •Implement new programs to keep existing staff up-to-date •Work independently in a field-based role •Manage multiple tasks •Excellent written and oral communication skills •Strong presentation experience •Enthusiastic, ability to motivate others.
